Forms to Bring

Please fill out the following forms completely and correctly

ie. High Adventure forms should be signed by a medical professional. Swim forms should be filled out by a certified lifeguard or swim instructor.

 Please note, each person attending camp should have 5 forms.

If you do not have all your forms or if they are not filled out completely, you'll be asked to leave camp to have forms completed properly prior to returning.

Medical History Form- Everyone needs one

Request to administer medicine form - everyone needs this even if you do not have medicines you take. We cannot give over the counter medicines without this.

Girl Behavior Form - all Girls need to have this filled out.
